Consent to Genetic Testing

I consent to Genetic testing. I understand that all materials, products, and results provided by EZ-Genetics LLC are provided for informational purposes only and that I should consult with a physician or healthcare provider regarding any question about fertility, pregnancy, or general health concerns. I also recognize that this does not replace the need for prenatal care, ultrasounds, or other pregnancy-related tests. I understand that EZ-Genetics LLC is not liable for any damages caused to me or anyone else as a result of using this product. I also certify that I am not using this product for reasons relating to gender selection or any other medical purpose. By purchasing from EZ-Genetics LLC, I agree to release, indemnify and hold harmless the company, all of its officers, and employees from all liabilities associated with the product(s) purchased. I understand that EZ-Genetics LLC is not liable for any action that might be taken by myself or anyone else as a result of using this service. I agree that any dispute or claim arising between myself and EZ-Genetics LLC will be resolved by binding arbitration, rather than in court. The Federal Arbitration Act and federal arbitration law apply to this agreement. We each agree that any dispute resolution proceeding will be conducted only on an individual basis and not in a class, consolidated or representative action.
